A. An end panel is simply your prescription information that is printed on the side of the contact lens box. To submit your rebate, we require you to "peel off" the end panel(s) and mail them to us. We've made them easy to remove so that the box remains intact.
A. The number of end panels you have to submit is located on the rebate form given to you by your eye care professional. The number can be different depending on the eligible lens purchase and rebate offer(s) available to you.
Q. What is the difference between the purchase date(s) and the postmark date?
A. All promotions have expiration dates -- your lenses must be purchased within 90 days of your initial eye exam and your rebate submission (including required documents) must be postmarked no more than 60 days after your lenses were purchased.
Q. How do I know which products, contact lenses or brands are eligible for rebates?
A. The rebate form that you received from your eye care professional shows a complete list of all of the products, contact lenses and brands that qualify—we do not allow product substitutions of any kind.
Q. I completed the online prequalification but I lost my confirmation page that I have to mail in with my other documents, how do I get another one?
A. It’s easy to re-print your confirmation page. Go to Rebate Status and type in your Name and Zip Code, it’s okay if you don’t have your Tracking Number. When it retrieves your form with all the information, you’ll be able to click on the Tracking Number link provided and print.
Q. My rebate form doesn’t have a code, what do I do?
A. The first step to prequalifying online is to enter your rebate code. If you don’t have a rebate code there is information and links provided right there that will generate a code for you on the spot—no waiting.
