Rebate Questions

Q. Exactly what do I need to submit to get my rebate?

A. In a single envelope you will need to mail us 5 documents:
  1. Completed prequalification form
  2. Rebate form given to you by your eye care professional
  3. Original eye exam receipt with the date circled (must be within 90 days of purchase)
  4. Original receipt for the contact lenses that you purchased (must be within 60 days of purchase)
  5. Peel off your prescription information or "end panels" located on the side of the box your lenses came in

You will receive the mailing address after you complete the online prequalification.

Q. Where do I get another "original" copy of the receipt or invoice if I can't find mine?

A. Most retailers and eye care professionals will be able to re-print a copy of the receipt for you, especially if you purchased with a credit card.

Q. What if I can't produce any receipts?

A. Unforunately, we will have to reject your rebate submission.

Q. What is an "end panel"?

A. An end panel is simply your prescription information that is printed on the side of the contact lens box. To submit your rebate, we require you to "peel off" the end panel(s) and mail them to us. We've made them easy to remove so that the box remains intact.

Q.How many end panels do I have to submit?

A. The number of end panels you have to submit is located on the rebate form given to you by your eye care professional. The number can be different depending on the eligible lens purchase and rebate offer(s) available to you.

A. Unfortunately, we will have to reject your rebate submission.

Q. I want to make sure I have the correct form, what does my rebate form from my eye care professional look like?

A. Here is a sample form. If you are unsure, check with your eye care professional to make sure they gave you the correct paperwork.

Q. What is the difference between the purchase date(s) and the postmark date?

A. All promotions have expiration dates -- your lenses must be purchased within 90 days of your initial eye exam and your rebate submission (including required documents) must be postmarked no more than 60 days after your lenses were purchased.

Q. I forgot to mail my rebate submission in on time, what do I do?

A. Unfortunately, we are unable to extend the date. Please stay in touch for future offers.

Q. How do I know which products, contact lenses or brands are eligible for rebates?

A. The rebate form that you received from your eye care professional shows a complete list of all of the products, contact lenses and brands that qualify—we do not allow product substitutions of any kind.

Q. I completed the online prequalification but I lost my confirmation page that I have to mail in with my other documents, how do I get another one?

A. It’s easy to re-print your confirmation page. Go to Rebate Status and type in your Name and Zip Code, it’s okay if you don’t have your Tracking Number. When it retrieves your form with all the information, you’ll be able to click on the Tracking Number link provided and print.

Q. I’ve mailed in my rebate submission with all of my documents, how do I check the status of my rebate?

A. You can track the status of your rebate submission anytime by Name, Zip Code or Tracking Number.

Q. How long can it take for my rebate submission to be processed?

A. It can take up to 60 days for processing and delivery of your rebate reward.

Q. I checked my rebate status and it’s telling me that I need to submit additional information. What do I need to do to get my rebate?

A. If your rebate submission is missing any information you will receive a notification by mail describing the reason. Follow the instructions given or contact us to help get the situation resolved.

Q. My rebate form doesn’t have a code, what do I do?

A. The first step to prequalifying online is to enter your rebate code. If you don’t have a rebate code there is information and links provided right there that will generate a code for you on the spot—no waiting.
